I give permission for this student to submit an idea or project to Thrive Labs.

I give Thrive Labs permission to review the submitted information, uploaded files, pitch deck, and project materials for application review, judging, mentorship, feedback, and program administration.

I confirm that the student’s submission is original work or properly credits any teammates, sources, tools, templates, or outside help used.

I understand that the student should not submit private, sensitive, confidential, medical, financial, password-protected, or unsafe information in the project materials.

I understand that submitting this form does not guarantee acceptance, prizes, awards, funding, mentorship, or participation in future rounds.

I understand that Thrive Labs may contact the parent/guardian and/or student by email about this submission.

I understand that Thrive Labs will use the submitted information only to review applications, communicate about the program, organize submissions, provide feedback, and manage the Innovation Series. Thrive Labs will not sell student information.

I understand that uploaded files and links must be appropriate, safe, related to the student’s project, and free of private, sensitive, confidential, medical, financial, password-protected, unsafe, or harmful content.

I agree to complete this permission form electronically and understand that my electronic signature and submitted responses may be kept as a record of my consent.
